Informational Materials

Materials are available to educate patients and those at-risk.  Each year, the National Kidney Foundation of Wisconsin provides brochures and other educational information to dialysis units, transplant centers, nephrology offices, other health professionals, and patients free of charge.

Many brochures and fact sheets are available online at www.kidney.org.

Printed materials that are not available online are provided free of charge by the National Kidney Foundation of Wisconsin; monetary contributions are welcomed to help defray the costs of purchasing, processing and shipping of these orders.

Use our Brochure Order Form to place an order for brochures.  Please do not use any other order form since many of the brochures included on older forms are no longer available.

FREE DVD

Medical ID Jewelry Information
Patients with chronic kidney disease,diabetes or high blood pressure, those on dialysis, transplant recipients and living donors are all eligible to receive FREE medical identification jewelry. Download the Medical ID Jewelry Order Form. Allow 4-6 weeks for processing.
