Information Materials

Each year, the National Kidney Foundation of Wisconsin provides thousands of dollars worth of brochures and other educational information to dialysis units, transplant centers, nephrology offices, other health professionals, and patients free of charge. Materials are available to educate patients and those at-risk of developing chronic kidney disease.

Many brochures and fact sheets are available online at www.kidney.org.

Printed materials that are not available online are provided free of charge by the National Kidney Foundation of Wisconsin; monetary contributions are welcomed to help defray the costs of purchasing, processing and shipping of these orders.

Medical ID Jewelry Information
Patients with chronic kidney disease, those on dialysis, transplant recipients and living donors are all eligible to receive FREE medical identification jewelry. Download the Medical ID Jewelry Order Form. Allow 4-6 weeks for processing.
