WAYS TO CONTRIBUTE
Your contributions to the NKFW are key to the success of the Foundation. A few of the many ways that you can help the children and adults of Wisconsin that are affected by chronic kidney disease, organ donation and transplantation are described below.
Shopping for Groceries
If you shop at a Pick N Save, stop by the Customer Service Desk to complete an "Advantage Card Application" form. Designate the National Kidney Foundation Of Wisconsin (#532200) as your charity of choice. Pick N Save will donate a % of whatever you spend on groceries to the NKFW.
Filling Your Gas Tank
The next time you fill up at a Kwik Trip, take a minute to complete the "Kwik Card Application form. Designate the National Kidney Foundation of Wisconsin (NKFW) as your 'charity of Choice'. You can also call 1-800-305-6666 if you have a Kwik Trip Card and want to add the NKFW as a charity. Kwik Trip, Inc. will donate 3 cents for every gallon of gas purchased and 5% of your eligible in-store grocery purchases to the NKFW.
Sending Flowers
Call 1-800-OUR-GIFT (1-800-687-4438) to order flowers for that special occasion or special someone. Tell the operator you want a special gift card enclosed announcing that a portion of the proceeds from your purchase will be donated to the National Kidney Foundation. You may also order online by visiting www.ourgift.com. There is prompt that will ask you whether you want to enclose a gift card and which 'affiliated charity' should be acknowledged.
Just find out who the employer campaign coordinator is for yur company and follow the steps below. If your company does not offer a workplace giving campaign or is not able to provide you with a Donor Choice campaign brochure, please call CHC Wisconsin at 414.933.4216.

You can help the National Kidney Foundation of Wisconsin continue to fight chronic kidney disease through your workplace United Way Campaign’s Community Donor Choice Campaign option.
It's as Easy as 1-2-3 Through Simple Payroll Deduction
1. Ask for a United Way of Greater Milwaukee Community Campaign Donor Choice Program brochure AND a separate United Way Pledge Form. In the United Way of Greater Milwaukee Community Campaign Donor Choice Program brochure, you will find a listing of CHC Wisconsin member charities in addition to United Way agencies and programs. Look for the National Kidney Foundation of Wisconsin and make note of its unique 5-digit organization number. Complete the form in the brochure. Choose the “Donor Choice” option located in the lower portion of the form. Fill in the ‘National Kidney Foundation of Wisconsin’ in the Donor Choice section with its organization number.
2. Now, complete the United Way Pledge Form (a triplicate form 8 1/2 x 11”). To direct your gift to the National Kidney Foundation of Wisconsin you must place an “X” next to the “Donor Choice” option on that form. Be sure to sign and date the form to ensure your gift reaches your intended charity.
3. Attach the Donor Choice Form to the United Way Pledge Form and turn them in to your employer campaign coordinator.
If the United Way campaign at your company is conducted on-line, you will need to locate CHC Wisconsin to designate your contribution to the National Kidney Foundation of Wisconsin. If you have difficulty locating CHC Wisconsin and its member charities on-line please call CHC Wisconsin at 414.933.4216.
