Fact # 18:
For early detection of chronic kidney disease, ask your doctor to test your blood for creatinine, test your urine for protein and measure your blood pressure.

CONTRIBUTE

with "MEMORIALS" and "IN HONOR GIFTS"

Special people in our lives should be remembered and honored. Memorial envelopes are available from the National Kidney Foundation of Wisconsin office and can be sent upon request. Gifts are acknowledged to the family of the deceased and a contribution verification is sent to the donor. Mastercard, Visa and American Express are all accepted by the National Kidney Foundation of Wisconsin.

Life's special moments should also be cherished and celebrated. Consider a monetary gift to the National Kidney Foundation of Wisconsin to celebrate a special day like a transplant or wedding anniversary; graduation; birth; or new job.

How about that 'hard'to buy for' person? The NKFW sends a special acknowledgement to that special boss, co-worker, neighbor, friend or relative on your behalf.

Contribution envelopes are available by calling 1.800.543.6393.

No time to wait for the mail? You can also contribute online here.

What can your contribution do?

$10 provides one dialysis patient or transplant recipient with a free medical identification bracelet or necklace. Last year, 1,200 patients felt more secure knowing that emergency personnel would know about their medical condition.

$20 will help pay for one person to have a 'free' kidney screening offered by the NKFW.

$50 will pay for 75 educational brochures that may be sent to a dialysis center, community health clinic., transplant center or nephrologist's office.

$100 will be used to pay for the food, facility costs and educational materials for patients and their families to attend one of the NKFW's educational programs.
