Brochures and Fact Sheets
As a healthcare professional, you know how important it is to have current information.

Each year, the National Kidney Foundation of Wisconsin provides thousands of dollars worth of brochures and other educational information to dialysis units, transplant centers, nephrology offices and other health professionals, free of charge. Materials are available to educate patients and those at-risk of developing chronic kidney disease.

Many brochures and fact sheets are available online at www.kidney.org .

Printed materials that are not available online are provided free of charge by the National Kidney Foundation of Wisconsin; monetary contributions are welcomed to help defray the costs of purchasing, processing and shipping of these orders.

Use our 2012 Materials Order Form (PDF) to place an order for brochures

Medical ID Jewelry Information
