Information Brochures for Patients With CKD

There is a wide variety of information available from the National Kidney Foundation of Wisconsin to help you when you have Chronic Kidney Disease (CKD). The brochures described below are just a few of the resources available. Here's a list of the 2009 Brochures available at the NKFW. Please feel free to call the NKFW to obtain copies.

Four new Medicare Part D brochures developed by the Kidney Medicare Drugs Awareness and Education Initiative, which is being led by the National Kidney Foundation.  The initative is a service of the kidney community, designed to provide timely, consistent, reliable, and up-to-date information about Medicare and Medicare Prescription Drug Coverage (Part D).

What You Should Know About Medicare Prescription Drug Coverage If...
       - You Have a Kidney Transplant
       - You Are Undergoing In-Center Hemodialysis
       - You Are Undergoing Home Hemodialysis or Peritoneal Dialysis
       - You Have Chronic Kidney Disease

         Choosing A Treatment For Kidney Failure

This brochure provides general information to help you and your family make this decision. You should discuss the pros and cons of each treatment option with your family, doctor, and your healthcare team. They will help you decide what is best for you.
